ABSTRACT

Introducción. Las malformaciones linfáticas quísticas, también llamadas linfangiomas quísticos, aparecen muy raramente de forma aislada en el hígado. Casos clínicos. Se presentan dos pacientes femeninas de edad preescolar con marcada hepatomegalia, dependiente de lesiones quísticas multitabicadas, secundarias a malformación linfática quística gigante del hígado, que fueron tratadas en el Hospital Pediátrico Universitario William Soler, La Habana, Cuba. Resultados. En ambos casos el diagnóstico se apoyó en los estudios de imágenes, la laparoscopia y el análisis histopatológico. En un caso el tratamiento fue la hepatectomía derecha, mientras que en el otro se empleó la escleroterapia, ambas con evolución favorable. Conclusión. A pesar de su rareza, este diagnóstico no debe obviarse ante un paciente pediátrico con lesiones hepáticas quísticas. El tratamiento de elección es la resección quirúrgica, pero su indicación y envergadura debe valorarse de forma individualizada


Introduction. Cystic lymphatic malformations, also called cystic lymphangiomas, are very rarely found in the liver. Clinical cases. Two pediatric female preschool-age patients. presented with hepatomegaly due to multi-septated cystic lesions of the liver, who received treatment at Hospital Pediátrico Universitario William Soler, La Habana, Cuba. Results. We report two pediatric cases with giant cystic lymphatic malformation of the liver. In both cases, the diagnosis were based on imaging, laparoscopy and pathology. In one case the treatment was right hepatectomy, whereas in the other, sclerotherapy was performed, both with a favorable outcome. Conclusion. Despite its rarity, this diagnosis should be considered in pediatric patients with hepatic cystic lesions. The recommended treatment is surgical resection, but its indication and extent should be assessed individually for each patient.

ABSTRACT Lymphangiomas are rare benign tumors that mainly involve the head and neck region in pediatric patients. Lymphangiomas of the small bowel mesentery in adults are rarer. We present two cases of mesenteric lymphangioma with acute abdominal pain on presentation. Case 1: A 38-year-old female presented with abdominal pain, vomiting, fever, and difficult evacuation. On abdominal examination, she had an ill-defined, tender lump, and radiological findings raised a possibility of perforation peritonitis. Thus, exploratory laparotomy was planned. Per-operatively, a mesenteric mass was found, which, on histopathological evaluation, was found to be a mesenteric lymphangioma involving the bowel. Case 2: A 27-year-old male presented with abdominal pain and difficult evacuation. Radiological evaluation revealed a multilobulated lesion involving the mesentery and with differential diagnoses of mesenteric fibromatoses and inflammatory pseudotumor. Histopathological assessment of the resected mass revealed a lymphangioma that was limited to the mesentery. Owing to their rarity and non-specific presentation, mesenteric lymphangiomas are often misdiagnosed on clinical examination and imaging. Thus, histopathological examination is the gold standard to reach a definitive diagnosis.

ABSTRACT Lymphatic malformation is a rare orbital tumor that used to be treated surgically, with high complication rates, or recently with intralesional bleomycin injection. We report for the first time the histopathological changes of eyelid lymphatic malformation after water-soluble intralesional bleomycin injection in a 20-year-old woman who had unsuccessful orbital surgical debulking during childhood. The changes confirmed the assumption of fibrosis induced by intralesional bleomycin injection. The minimal bleeding during surgical intervention made it much easier than the usual lymphatic malformation bloody procedure, without postoperative recurrences and with favorable aesthetic outcomes.


RESUMO A malformação linfática é um tumor orbital raro que costumava ser tratado cirurgicamente, com alta taxa de complicações. Mais recentemente, passou a ser tratado com uma injeção intralesional de bleomicina. Este é o primeiro relato sobre as alterações histopatológucas da malformação linfática palpebral após uma injeção intralesional de bleomicina hidrossolúvel em uma mulher de 20 anos de idade que sofreu uma cirurgia malsucedida de debulking orbital durante a infância, confirmando a suposição de fibrose induzida por injeções intralesionais de bleomicina. O sangramento mínimo durante a intervenção cirúrgica tornou esta muito mais fácil que o procedimento sangrento habitual, sem recidivas pós-operatórias e com desfechos estéticos favoráveis.

ABSTRACT

Retroperitoneal cystic lymphangiomas (RCL) are rare benign tumors of the lymphatic system. They account for less than 1% of all lymphangiomas. Surgical resection is the recommended treatment option; however, obtaining a pre-operative diagnosis is often difficult and, in most cases, the final diagnosis is only possible following histological assessment of the surgical specimen. This report describes a case of RCL in a 58-year-old female cirrhotic patient who presented to our center with dull aching abdominal pain and distension. To our knowledge, this is the first case of a RCL in a cirrhotic patient reported in the literature.


Los linfangiomas quísticos retroperitoneales (LQR) son tumores benignos raros del sistema linfático y representan menos del 1% de todos los linfangiomas. La resección quirúrgica es la opción de tratamiento recomendada; sin embargo, obtener un diagnóstico preoperatorio suele ser difícil y, en la mayoría de los casos, el diagnóstico final solo es posible tras la evaluación histológica de la pieza operatoria. En este estudio, describimos un caso de un LQR en una paciente cirrótica de 58 años que se presentó en nuestro centro con dolor abdominal sordo y distensión. Hasta donde sabemos, este es el primer caso de LQR en un paciente cirrótico reportado en la literatura.

Los linfangiomas son malformaciones linfáticas benignas infrecuentes; se manifiestan principalmente en la infancia. Pueden desarrollarse en cualquier zona del cuerpo, más frecuentemente cabeza y cuello, siendo su compromiso faríngeo extremadamente poco común. Dada su clínica inespecífica se requiere un análisis anatomopatológico preciso para concretar un correcto diagnóstico. Presentamos el caso de una mujer de 40 años con historia de disfagia y odinofagia de un año de evolución cuyo estudio imagenológico y con nasofibroscopía evidenció un tumor redondeado alojado en seno piriforme derecho, la biopsia excisional confirmó el diagnóstico de linfangioma. Se debe considerar el diagnóstico de linfangioma dentro de los diagnósticos diferenciales de tumores en faringe y concientizar su presencia en pacientes adulto, para ofrecer el manejo óptimo basado, ya sea en escisión quirúrgica completa, para evitar la recurrencia asociado, o al uso de sustancias esclerosantes.


Lymphangiomas are infrequent benign lymphatic malformations, with clinical manifestations mainly at birth or in childhood. They can develop in any area of the body, most frequently the head and neck, and their pharyngeal involvement is extremely rare. Given its nonspecific symptoms, it requires a precise pathological analysis to make a correct diagnosis. We present the case of a 40-year-old woman with a 1-year history of dysphagia and odynophagia whose imaging study and nasofibroscopy revealed a rounded tumor lodged in the right pyriform sinus. The excisional biopsy confirmed the diagnosis of lymphangioma. The diagnosis of lymphangioma should be considered within the differentials of tumors in the pharynx and awareness of its presence in adults to offer optimal management based on complete surgical excision to avoid recurrence or management with use of sclerosing substances.

ABSTRACT Hemangiolymphangioma is a very rare vascular malformation that develops as a combination of dilated venous and lymphatic vessels. We describe an unusual case of hemangiolymphangioma of the tongue affecting an adult man who complained of an uncomfortable, slowly progressing exophytic irregular dark red-violaceous nodular mass on his tongue that impaired speech and swallowing for two weeks. The clinical differential diagnoses were Kaposi's sarcoma and a COVID-19-related lesion. A complete blood count and serology for HIV-1 and 2 and RT-PCR for COVID-19 were requested and results were negative. An incisional biopsy was performed. Microscopically, the lesion exhibited several dilated vessels lined by normal-appearing endothelial cells, some filled with prominent intravascular erythrocytes and others containing proteinaceous eosinophilic material resembling lymphatic vessels, in close association with hyperkeratosis, papillomatosis, and acanthosis. From immunohistochemical analysis, most vessels were found to be CD34 positive, some highlighted by α-SMA, whereas D2-40 was focal. Positive staining for some lymphatic and blood vessel markers, i.e., D2-40 and CD34, respectively, indicates a mixed derivation of the lesion. HHV-8 was negative. Clinical features, the congested blood vessels with ectasia in intimate association with hyperplastic epithelium, and the immunohistochemical profile supported the final diagnosis of oral hemangiolymphangioma. The patient underwent minimally invasive surgical excision with no intercurrences. After 18 months of follow-up, there were no signs of relapse.

Introduction: Cystic hygroma (CH) is a rare congenital anomaly of the lymphatic system. It is characterized by cystic lesions predominantly in the fetal neck and its prenatal diagnosis has been associated with increased perinatal mortality, aneu-ploidy, and congenital malformations. Case presentation: Two cases of cervical cystic hygroma diagnosed during the second trimester of gestation are presented, one of them associated with bilateral clubfoot. Both fetuses underwent karyotyping by amniocentesis, which established that both were euploid (46 XY and 46 XX), as well as fetal nuclear magnetic resonance imaging that showed no associated major malformations. In the interdisciplinary follow-up performed 1 year after birth, no findings consistent with genetic syndromes or neurodevelopmental alterations were observed in either of the 2 cases. Conclusions: CH is a marker of poor fetal prognosis; however, euploid fetuses with this condition have a better prognosis if their lesion resolves, do not progress to hydrops fetalis, and do not present other associated malformations. Euploid fetuses with CH require specific genetic studies for RASopathies, such as Noonan syndrome, which were not available in the clinical approach of the 2 cases presented; however, typical postnatal characteristics of the disease were not evident in the clinical genetic evaluation.


Introducción. Los higromas quísticos (HQ) son anomalías congênitas poco frecuentes del sistema linfático que se caracterizan por lesiones quísticas, de predominio en el cuello. Su diagnóstico prenatal se ha asociado con un aumento de mortalidad perinatal, aneuploidías y malformaciones congênitas. Presentación de los casos. Se presentan dos casos de HQ cervicales diagnosticados durante el segundo trimestre de gestación, uno de ellos asociado a pie equino varo bilateral. En ambos fetos se practicó amniocentesis para realizar prueba de cariotipo, que permitió establecer que ambos eran euploides (46XY y 46XX), y resonancia magnêtica nuclear fetal, que no mostró malformaciones mayores asociadas. En el seguimiento interdisciplinario realizado 1 año después del nacimiento no se observaron hallazgos compatibles con síndromes genêticos ni alteraciones en el neurodesarrollo en ninguno de los 2 casos. Conclusiones. El HQ es un marcador de mal pronóstico fetal; sin embargo, fetos euploides con esta condición tienen mejor pronóstico si presentan resolución de la lesión y no desarrollan hidropesía fetal ni otras malformaciones asociadas. Los fetos euploides con HQ requieren estudios genéticos específicos para rasopatías, como el síndrome de Noonan, los cuales no estuvieron disponibles en el abordaje clínico en estos 2 casos; sin embargo, en la valoración por genética clínica no se hallaron características postnatales típicas de estas afecciones.

ABSTRACT

Los linfangiomas orbitarios son malformaciones vasculares benignas, de crecimiento lento, abortivas y no funcionales, que se presentan principalmente en la primera década de la vida. Las opciones terapéuticas en estos casos presentan una resolutividad limitada, algunos tratamientos suelen ser agresivos y provocar daños del aparato visual. Se presenta un caso de una paciente femenina de 6 años de edad atendida por proptosis del ojo izquierdo a la que se le realizó el diagnóstico clínico-imagenológico de linfangioma de la órbita, con el objetivo de mostrar el resultado alcanzado en el manejo de la misma mediante el uso del sildenafilo por vía oral, modalidad terapéutica en estudio a nivel mundial en el tratamiento de estas afecciones. El tratamiento con sildenafilo en el linfangioma orbitario demostró ser eficaz en la mejoría del cuadro clínico y por imágenes. Durante el tratamiento no se reportaron reacciones adversas(AU)


Orbital lymphatic malformations are benign, slow-growing, abortive, nonfunctional vascular malformations that occur mainly in the first decade of life. Therapeutic options in these cases present limited resolution, some treatments are usually aggressive and cause damage to the visual apparatus. We present a case of a 6-year-old female patient treated for proptosis of the left eye. The clinical-imaging diagnosis of lymphangioma of the orbit was made to show the results achieved in its treatment through the use of oral sildenafil, a therapeutic modality under study worldwide in the treatment of these conditions. The treatment with sildenafil in orbital lymphangioma proved to be effective in the improvement of the clinical and imaging picture. No adverse reactions were reported during treatment(AU)

Los linfangiomas son malformaciones benignas del sistema linfático que afectan con mayor frecuencia a los niños y en raras ocasiones a adultos. Presentamos el caso de una mujer de 36 años, con cuadro clínico de 6 meses de dolor abdominal difuso sin irritación peritoneal, y asociado a adinamia. Como parte de los estudios, se solicitó una tomografía axial computarizada con contraste de abdomen, que reveló múltiples lesiones esplénicas hipodensas hipovasculares, con sospecha diagnóstica de lesiones linfoproliferativas. Por tal motivo se solicitó una esplenectomía diagnóstica, que finalmente mostró hallazgos macroscópicos, microscópicos y de inmunohistoquímica (positividad de las lesiones quísticas para CD31 y D2-40) compatibles con linfagiomatosis esplénica difusa.


Lymphangiomas are benign malformations of the lymphatic system. They most often affect children and rarely adults. We present the case of a 36-year-old woman with a 6-month clinical picture of diffuse abdominal pain without peritoneal irritation associated with adynamia. As part of the studies, a computerized axial tomography with abdominal contrast was requested, which revealed multiple hypovascular hypodense splenic lesions with suspected diagnosis of lymphoproliferative lesions. For this reason, a diagnostic splenectomy was requested, which showed macroscopic, microscopic and immunohistochemical findings (positivity of cystic lesions for CD31 and D2-40) compatible with diffuse splenic lymphagiomatosis

Se presenta dos casos de gestantes en el tercer trimestre de gestación referidas para manejo de tumor cervical fetal con obstrucción de la vía respiratoria. Los casos fueron programados para cirugía fetal EXIT (ex-utero intrapartum treatment) que permitió la intubación orotraqueal intraparto controlada evitando el período de hipoxia y potencial asfixia que ocurrirían con el procedimiento neonatal convencional. Los recién nacidos fueron operados posteriormente por los cirujanos de cirugía de cabeza y cuello para la reducción de la masa cervical. Se sugiere plantear este procedimiento en los casos en que se sospeche obstrucción severa de la vía aérea y referirlos a centros que cuenten con una unidad de cirugía fetal con equipo médico multidisciplinario.


Two cases of pregnant women in the third trimester of gestation referred for management of fetal cervical tumor with airway obstruction are presented. The cases were scheduled for EXIT (ex-utero intrapartum treatment), fetal surgery which allowed controlled intrapartum orotracheal intubation avoiding the period of hypoxia and potential asphyxia that would occur with the conventional neonatal procedure. The neonates were subsequently operated on by head and neck surgeons for reduction of the cervical mass. It is suggested to consider this procedure in cases where severe airway obstruction is suspected and to refer them to centers that have a fetal surgery unit with a multidisciplinary medical team.

Lymphangioma circumscriptum (LC) is a form of lymphatic hamartoma, which mainly involves the skin and subcutaneous tissues. The mucosa of the mouth, tongue, groin, trunk, and axilla are the most common locations of LC, and the scrotum is a rare site of occurrence. Here we report a case of LC of scrotum occurring in a 58-year-old man without any history of surgery and radiation. The man was presented with multiple small vesicles containing clear fluid in the scrotum and subsequent histopathology confirmed the diagnosis. We are presenting this case for its rare presentation.

Introducción. Los linfangiomas son anormalidades benignas del sistema linfático, que corresponden a dilataciones quísticas de estos vasos y se localizan especialmente en el cuello. Solo el 10 % de todas estas malformaciones se encuentran en el abdomen y presentan síntomas variables de acuerdo al tamaño y su ubicación especifica, siendo el dolor abdominal el principal síntoma. Métodos. Se presentan cinco pacientes pediátricos con malformaciones linfáticas abdominales. Se describen su cuadro clínico, localización, tratamiento y la experiencia en el manejo de dicha patología en un hospital de referencia. Resultados. Los métodos más apropiados para hacer una aproximación diagnóstica son la ecografía, la tomografía computarizada y la resonancia nuclear magnética. Dentro de las opciones descritas para el tratamiento están la farmacológica, la escleroterapia y la resección quirúrgica, tanto por vía abierta como por laparoscopia. Conclusión. Existe una variedad de métodos para realizar la resección de los linfangiomas abdominales, pero la cirugía sigue siendo la más efectiva, especialmente cuando se cuenta con la laparoscopia como una herramienta terapéutica.


Introduction. Lymphangiomas are benign abnormalities of the lymphatic system, which correspond to cystic dilations of these vessels and are located especially in the neck. Only 10% of all these malformations are found in the abdomen and present variable symptoms according to size and their specific location, with abdominal pain being the main symptom. Methods. Five pediatric patients with abdominal lymphatic malformations are presented. Their clinical presentation, location, treatment and experience in the management of this pathology in a referral hospital are described. Results. The most appropriate methods to make a diagnostic approach are ultrasound, computed tomography and magnetic resonance imaging. Among the options described for treatment are pharmacological, sclerotherapy and surgical resection, both open and laparoscopic. Conclusion. There are a variety of methods for resecting abdominal lymphangiomas, but surgery remains the most effective, especially when laparoscopy is used as a therapeutic tool.

Objective:To investigate the clinical features, diagnosis, treatment and prognosis of adult cervical lymphangioma.Methods:Twenty-five cases of adult cervical lymphangioma admitted at Department of Lymphatic Surgery of Beijing Shijitan Hospital from Feb 2017 to Jun 2020 were retrospectively analyzed.Results:Among the 25 cases,there were 5 males and 20 females, aged from 18 to 58 years. Radionuclide lymphoscintigraphy performed in 25 cases, increased radioactivity in the neck tumor area was seen in 8 cases; Direct lymphangiography performed in 17 cases,outlet obstruction of cervical thoracic duct was found in 14 cases. Twenty one cases were treated by operation and followed up for 6-47 months. No recurrence was found; 4 cases abandoned surgery, 1 case was lost to follow-up, and the other 3 cases were followed up for 16-48 months with tumor progresses.Conclusions:Adult cervical lymphangioma is lymphangiomalformation disease. Total resection of the tumor and avoiding damage to the thoracic duct is the key for successful management.

Objective:To analyze the correlation of fetal cervical cystic hygroma (CCH) with chromosomal and structural abnormalities and to assess the prognosis of CCH.Methods:This study retrospectively enrolled 70 fetuses with CCH diagnosed by prenatal ultrasound in the First Affiliated Hospital of Xi'an Jiao Tong University from July 2015, to December 2021. According to whether complicated by structural malformations or other anomalies, all the subjects were divided into the non-isolated and isolated CCH groups. The correlation of CCH and the gestational age at detection with chromosomal and structural abnormalities were analyzed and the prognosis of the cases were summarized using Chi-square test. Results:There were 34 isolated CCH (34/70, 49%) and 36 non-isolated CCH (36/70, 51%) among the 70 cases. In the non-isolated CCH group, there were eight cases (22%, 8/36) with abnormal heart structure, ten (28%, 10/36) with abnormal anterior abdominal wall, 16 (44%,16/36) with systemic edema and/or pleural effusion, one (3%,1/36) with craniocerebral abnormalities and one with holoprosencephaly and cardiac structural abnormalities. Eighteen out of 44 cases undergoing chromosome testing had chromosomal abnormalities, which were trisomy-18 ( n=6), trisomy-21 ( n=3), trisomy-13 ( n=3), 45,XO ( n=3), and chromosome segment duplication or deletion ( n=3). The detection rate of chromosome abnormality was higher in non-isolated CCH group comparing with isolated CCH group [59%(13/22) vs 23%(5/22), χ2=6.02, P=0.014]. There was no significant difference in the gestational age at the detection of CCH or proportion of women of advanced maternal age between the isolated and non-isolated CCH groups (both P>0.05). The ratios of isolated CCH cases with normal chromosome detected at the gestational weeks of 14-27 +6 was higher than those detected at 11-13 +6 weeks [62%(13/21) vs 17%(4/23), χ2=7.39, P=0.001]. Out of the 17 cases with isolated CCH and normal chromosomes, 12 were live births. One of the 12 cases still had a cystic mass with a diameter of 3 cm in the neck nine months after birth, and the other 11 cases had no mass at birth but one case died at the age of five months (hospitalized one week for neonatal edema),one case was found with anal atresia three days after birth and underwent operation and the remaining nine cases were normal during five months to six years follow-up. Conclusions:Non-isolated CCH is at a higher risk of chromosomal abnormalities. Isolated CCH cases detected later had higher rate of normal chromosome and often have a higher survival rate.

Resumen 22. El linfangioma circunscripto (LC) es una afección rara y benigna, caracterizada principalmente por la malformación de los vasos linfáticos de la piel. De etiología congénita o adquirida,esta última debido a causas secundarias como radioterapia, infecciones o procedimientos quirúrgicos. 23. Presentamos el caso de un paciente de 75 años de edad, con antecedente quirúrgico de fimosis a los 29 años, y posterior desarrollo de linfangioma circunscripto de glande diagnosticado 46 años después. El tratamiento definitivo planteado en interconsulta con Urología fue la resección quirúrgica, motivados por la escasa frecuencia de recidivas cuando se usa este método.


Summary 27. Circumscribed lymphangioma (CL) is a rare and benign condition, characterized mainly by malformation of the lymphatic vessels of the skin. It has a congenital or acquired etiology, the latter due to secondary effects such as radiotherapy, infections or surgical procedures. 28. We present the case of a 75-year-old patient, with a surgical history of phimosis at 29 years of age, and subsequent development of circumscribed lymphangioma of the glans penis diagnosed 46 years later. The definitive treatment proposed in consultation with Urology was surgical resection due to the low frequency of recurrences.

INTRODUCCIÓN. Los linfangiomas son una malformación infrecuente a escala mundial y constituye una preocupación para los padres del infante; está asociado a problemas estéticos y a posibles efectos deletéreos debido a la obstrucción o compresión de órganos vitales. Se ha descrito a la escleroterapia como la mejor opción de tratamiento. OBJETIVO. Determinar la eficiencia del tratamiento con bleomicina en linfangiomas en la población pediátrica de 0 a 18 años. MATERIALES Y MÉTODOS. Estudio transversal analítico retrospectivo. Población y muestra conocida de 20 datos de Historias Clínicas electrónicas de pacientes diagnosticados con linfangiomas y tratados con bleomicina en el Hospital de Especialidades Carlos Andrade Marín, desde enero 2015 a enero 2018. Criterios de inclusión: pacientes de 0 a 18 años de edad con diagnóstico de linfangioma mediante ecografía y angiotomografía computarizada. Criterios de exclusión: pacientes mayores de 18 años de edad o sospecha diagnóstica de linfangioma sin estudios de imagen, y/o que no acudieron a la cita de control, pacientes diagnosticados de linfangioma que no recibieron bleomicina para su tratamiento, niños con otras malformaciones vasculares. El análisis de datos se realizó en el programa estadístico International Business Machines Statistical Package for the Social Sciences. RESULTADOS. La mediana de edad en mujeres fue de 6,25 años y 3,8 años en hombres. 10 pacientes fueron hombres. El promedio de seguimiento fue de 26,86 +/- 16,78 meses. El tamaño promedio de los linfangiomas fue de 5,77 +/- 3,73 cm. La localización más frecuente fue cervical con un 52,38%. La mayoría fueron macronodulares con un 85,71%. La respuesta fue buena o excelente en el 81,00% de los casos con la primera infiltración y subió al 95,00% con la segunda y tercera, según requerimiento. CONCLUSIÓN. El tratamiento de los linfangiomas con bleomicina fue muy efectivo en la población estudiada.


INTRODUCTION. Lymphatic malformation is a rare malformation worldwide and is a concern for the parents of the infant; it is associated with aesthetic problems and possible deleterious effects due to obstruction or compression of vital organs. Sclerotherapy has been described as the best treatment option. OBJECTIVE. To determine the efficiency of bleomycin treatment in lymphangiomas in the pediatric population aged 0 to 18 years. MATERIALS AND METHODS. Retrospective analytical cross-sectional study. Population and known sample of 20 data from Electronic Medical Records of patients diagnosed with lymphangiomas and treated with bleomycin at the Carlos Andrade Marín Specialties Hospital, from January 2015 to January 2018. Inclusion criteria: patients aged 0 to 18 years with diagnosis of lymphangioma by ultrasound and computed angiotomography. Exclusion criteria: patients older than 18 years of age or diagnostic suspicion of lymphangioma without imaging studies, and/or who did not attend the control appointment, patients diagnosed with lymphangioma who did not receive bleomycin for treatment, children with other vascular malformations. Data analysis was performed in the statistical program International Business Machines Statistical Package for the Social Sciences. RESULTS. The median age in women was 6,25 years and 3,8 years in men. Ten patients were men. The average follow-up was 26,86 +/- 16,78 months. The average size of the lymphatic malformations was 5,77 +/- 3,73 cm. The most frequent location was cervical with 52,38%. Most were macronodular with 85,71%. The response was good or excellent in 81,00% of cases with the first infiltration and rose to 95,00% with the second and third, as required. CONCLUSION. The treatment of lymphangiomas with bleomycin was very effective in the population studied.

ABSTRACT

RESUMEN Los linfangiomas abdominales son tumores quísticos poco frecuentes en pediatría, representan el 1% de todos los casos de linfangiomas; los síntomas clínicos no son específicos, se descubren de forma incidental; actualmente el manejo quirúrgico es de primera elección, pero se deben valorar otras posibilidades terapéuticas como los agentes esclerosantes que, dependiendo del tamaño, la ausencia o no de sintomatología y las comorbilidades del paciente se convierte en otra opción terapéutica menos invasivo con el cual se logran buenos resultados; se describe los casos clínicos de dos neonatos con diagnóstico de linfangioma retroperitoneal, manejados con agentes esclerosantes (caso 1) y cirugía (caso 2), mostrando las diferentes opciones terapéuticas.


ABSTRACT Abdominal lymphangiomas are infrequent cystic tumors in pediatric patients, they represent 1% of all cases of lymphangiomas; clinical symptoms are not specific, they are usually discovered incidentally; Surgical management is currently the first-line treatment, but other therapeutic possibilities, such as sclerosing agents, must be evaluated. Depending on the size, the presence or not of symptoms and the patient's comorbidities, these alternatives become another less-invasive therapeutic option, with which good results may be achieved. Two clinical cases of neonates diagnosed with retroperitoneal lymphangioma are described, one was managed with sclerosing agents and the other with surgery, exemplifying different therapeutic options.

Introducción: El linfangioma quístico es un tumor benigno infrecuente del sistema linfático que afecta habitualmente a los infantes. La opción terapéutica ideal es la extirpación quirúrgica. Objetivo: Sistematizar contenidos esenciales relacionados con el diagnóstico y tratamiento del linfangioma quístico. Métodos: Se realizó la búsqueda y el análisis de la información en un período de 10 años (2010-2020). Se emplearon las palabras clave: "linfangioma quístico" y "malformación linfática congénita quística", en español e inglés. Se hizo la revisión bibliográfica en un total de 62 artículos publicados en las bases de datos y bibliotecas electrónicas científicas de la salud: PubMed, Google Académico, Pubmed, Medline, Biblioteca Virtual en Salud, LILACS y SciELO, mediante el gestor de búsqueda y administrador de referencias EndNote; de ellos se seleccionaron 50 relevantes para el objetivo de la revisión. Resultados: La información se estructuró en los siguientes aspectos: sinonimia y antecedentes históricos, epidemiología, clasificación, patogenia, bases esenciales para el diagnóstico, tratamiento, complicaciones y pronóstico. Se identificaron controversias en cuanto a la terapéutica, y se mostraron las imágenes de los infantes diagnosticados y tratados por la autora en su colaboración médica en Angola en 2018. Conclusiones: Se sistematizan las bases esenciales para el diagnóstico y tratamiento del linfangioma quístico para que el cirujano general que, en su desempeño profesional, brinda asistencia médico quirúrgica a infantes fuera de Cuba, se empodere de las especificidades de este tumor linfático. También se revela como factible su resección quirúrgica íntegra para evitar recidivas, discapacidad y mejorar la calidad de vida del afectado(AU)


Introduction: Cystic lymphangioma is a rare benign tumor of the lymphatic system that usually affects infants. The ideal therapeutic option is surgical removal. Objective: Systematize essential contents related to the diagnosis and treatment of cystic lymphangioma. Methods: The search and analysis of information was carried out over a period of 10 years (2010-2020). The keywords "cystic lymphangioma" and "cystic congenital lymphatic malformation" were used in Spanish and English. The bibliographical review was carried out in a total of 62 articles published in the databases and electronic scientific health libraries: PubMed, Google Scholar, Pubmed, Medline, Virtual Library in Health, LILACS and SciELO, through the search and reference manager called EndNote; of these, 50 that were important to the objective of the review were selected. Results: The information was structured in the following aspects: synonym and historical backgrounds, epidemiology, classification, pathogenesis, essential bases for diagnosis, treatment, complications and prognosis. Therapeutic disputes were identified, and images of infants diagnosed and treated by the author in her medical collaboration in Angola in 2018 were shown. Conclusions: The essential bases for the diagnosis and treatment of cystic lymphangioma are systematized, so that the general surgeon who, in his-her professional performance, provides surgical medical assistance to infants outside Cuba is informed on the specificities of this lymphatic tumor. Its full surgical resection is also revealed as feasible to prevent recurrence, disability and to improve the quality of life of the affected person(AU)

ABSTRACT

INTRODUCCIÓN: El linfangioma mesentérico es un tumor quístico congénito, benigno y raro de los vasos linfáticos, que se presenta en menos del 5% de los casos a nivel abdominal. Se presentan con mayor frecuencia durante la infancia. Esta patología se debe considerar como diagnóstico diferencial de abdomen agudo y masas abdominales pediátricas. CASO CLÍNICO: Niño de 6 años de edad, presentó cuadro de dolor abdominal y peritonismo. Presentó febrícula, signo de rebote positivo, además leucocitosis y neutrofilia. Ecografía evidenció abundante líquido libre en abdomen y pelvis. EVOLUCIÓN: Se realizó laparotomía exploratoria, con hallazgo de masa quística dependiente de epiplón mayor, la misma que se resecó. La histopatología fue negativa para malignidad y compatible con linfangioma quístico, la inmunohistoquímica con marcador D2-40 positi-vo. El paciente evolucionó favorablemente sin complicaciones quirúrgicas y sin evidencia de recurrencia hasta el año de seguimiento. CONCLUSION: El linfangioma quístico mesentérico puede debutar con sintomatología de abdomen agudo, puede también tener una evolución crónica. La resección completa es el tratamiento de elección, actualmente se realizan procedimientos mínimamente invasivos, con resultados favorables. La ecografía es útil para realizar un seguimiento a largo plazo.(AU)


BACKGROUND: Mesenteric lymphangioma is a rare, benign congenital cystic tumor of the lym-phatic vessels, which occurs in less than 5% pf the cases at the abdomen. They appear most often during childhood. This tumor should be considered as a differential diagnosis of acute abdomen and other abdominal masses. CASE REPORT: A 6-year-old boy presented with abdominal pain and peritonism. He presented a low-grade fever, a positive rebound sign, as well as leukocytosis and neutrophilia. Ultrasono-graphy showed abundant free fluid in abdomen and pelvis. EVOLUTION: Exploratory laparotomy was performed, finding a cystic mass dependent on the greater omentum, this mass was resected. Histopathology was negative for malignancy and compatible with cystic lymphangioma, immunohistochemistry was positive for D2-40 marker. The patient evolved favorably without surgical complications and without evidence of recurrence up to one year of follow-up. CONCLUSION: Mesenteric cystic lymphangioma can present with symptoms of acute abdomen, it can also have a chronic evolution. Complete resection is the treatment of choice, currently minimally invasive procedures are performed with favorable results. Ultrasound is useful for long-term follow-up.(AU)
